A Study on Comparison of the Indirect nursing care of Two hospitals

Abstract

This study selected each 8 medical, surgical nursing; units of hospitals applied computer system and other non-computerized hospitals. And by using Patient Classification Tool and Indirect Nursing Activities Checklist composed of 5 domains and 15 items :modified through pretest of upper nursings units, classified 751 inpatients of the selected hospitals and investigated 134 datas of indirect nursing time recorded by oneself of 83 nurses working in same hospitals with the exception of head nurses during the from February 25th to 26th, 1998.
The results of study were summarized as follows.
1. In genernal characteristics, mean age of nurses was 27.0 years old in case of Computerized hospital (below A hospital) and 28.1 in. Non-computerized hospital (below B hospital). And the mean of age was not significantly differed in two hospitals (t=-1.047, P= .823). 78% nurses of A hospital were single and 69.7% in B hospital. In educational background, 56% had the diploma degrees in A hospital, on the other hand 51.5% had the diploma degrees in B hospital. In total career, the mean value were 3 years 4 months in A hospital and 4 years 6 months in B hospital. And the mean of total career was not significantly differed in two hospital (t=-2.439, P= .074).
2. In the distribution of patient classification, I class(simple case) was 37.7%, 11class(less serious case) was 34.0%, Mclass(serious case) was 20.8%, and Nclass(dangerous case) was 7.5% in A hospital. In B hospital, I class was 47.4%, Eclass was 31.6%. Mclass was 15.8%, and Nclass was 5.2%. And it was significantly differed in the distribution of patient classification of two hospitals (Table 2-1).
3. In domain of nursing record, the mean time of recording related patients care was not significantly differed in two hospitals (t=-1.213, P= .227). The mean time of recording of ward management was not significantly differed in two hospitals (t=-1.585, P= .115). And, the mean time of management of medical record was significantly differed in two hospitals (t=-4.174, P= .000).
4. In domain of confirmation, the mean time of confirmation of X-Ray films, specimens and medical records was significantly differed in two hospitals (t=-4.399, P= ..000). The mean time of confirmation of Dr¢¥s order was not significantly differed in two hospitals (t= .388, P= .699). And the mean time of taking over the duties was significantly differed in two hospitals (t=-2.611, P= .010).
5. In domain of environmental management, the mean time of management of drugs was significantly differed in two hospitals (t=-2.312, P=: .022). The mean time of management of materials was not significantly differed in two hospitals (t = -. 262, P= .793). The mean time of management of facilities was not significantly differed in two hospitals (t = 1.002, P= .318). And the mean time of management of ward cleasing was not significantly differed in two hospitals(t=1.366, P= .174).
6. In domain of communication, the mean time of discussion for patient complaints was significantly differed in two hospitals (t = -2.930. P= .004). The mean time of education and training was not significantly differed in two hospitals(t= .728, P= .468). And the mean time of the affairs related telephone and beeper was not significantly differed in two hospitals (t=-.186. P= .853).
7. In domain of delivery. the mean time of delivery of materials was significantly differed in two hospitals (t=-2.277, P= .024). And the mean time of participation in a delivery of meal was not significantly differed in two hospitals (t=-1.021, P= .309).
8. The mean of break time was not significantly differed in two hospitals (t=-1.374. P= .172). And the mean of total indirect nursing time was not significantly differed in two hospitals (t=- .751, P= .454).
In the conclusion, continuous effort is necessary to improve the quality of nursing and to construct total nursing information system for users convenience. The studies for understanding effectiveness of computerized nursing work should be extended to all hospitals where computer system have applied. It is demanded to certify patients perception about the changes from appling computer system on nursing.
